EVV stands for Electronic Visit Verification. It’s a way for home healthcare and personal care providers to prove that the provider delivered the agreed-upon service(s) documenting at minimum six key data points.

1. Patient name
2. Date of Service
3. Location of Service
4. Type of Service Performed
5. Healthcare Provider’s Name (Nurse or Aide Name)
6. Start and End Time (of Care)
